GUIDE TO MOTORCAR CLAIMS
I. WHAT TO DO IN CASE OF VEHICULAR ACCIDENT
A. Notify the nearest Police Officer, Police Station for assistance.
B. Take down the description, name and address of the registered owner of the other vehicle; names of the other driver/witness and their addresses.
C. If possible, get the name of the insurance company and policy number of the other vehicle.
D. Whenever possible, never leave your vehicle unattended especially in remote places without proper precautions to prevent further damage or theft.
E. Immediately report the accident to Philippines First Insurance Co., Inc.
II. DOCUMENTATIONS – documents that would be required from the insured and/or authorized driver are as follows in respective of the nature of the claim:
FOR OWN DAMAGE CLAIM (Damage to the Insured Vehicle)
1. Photocopies of the following:
a. Policy and Official Receipt
b. Certificate of Registration and OR
c. Driver’s License and OR
2. Original Copy of Police Report &/Or Notarized Affidavit
3. Repair Estimate
4. Photos of the Insured Vehicle showing its license plate and the extent of the damage to vehicle
5. Stencils of Motor and Serial Numbers
FOR THIRD PARTY PROPERTY DAMAGE
1. Photocopies of the following:
a. Policy and Official Receipt
b. LTO Certificate of Registration and its Official Receipt
c. Driver’s License and its Official Receipt
d. Third Party’s Certificate of Registration and its Official Receipt
e. Third Party’s Driver’s License and its Official Receipt
f. Original Certificate of No Claim from Third Party’s Insurer
2. Original Copy of Police Report &/Or Driver’s Affidavit executed by the Insured’s driver.
3. Repair Estimate
4. Photos of the Third Party Vehicle showing its license plate and the extent of the damage of the same
5. Stencils of Motor and Serial Numbers
Note: If the insured unit will be repaired at our Accredited Repair Shops 3, 4 & 5 can be provided by the same. If to be, cash settled insured must provide all the necessary documentation FOR THIRD PARTY BODILY INJURY/DEATH CLAIM
